By attending your consultation, you confirm that you have read and agree to the terms of this agreement.

We will use your personal information as set out in our privacy policy (see separate document).
General
Our expectations from you:
You should provide your medical history and any relevant test results prior to your consultation, if tests have been completed by another clinic. If the tests are more than six months old, we advise some of them to be repeated with us for separate fees.
Please note that if you do not, or are unable to provide your full medical history, or have not completed the mandatory tests which vary depending on your treatment, we will not be able to provide personalised treatment recommendations. A further consultation will be required, and additional fee is payable.
Following your consultation, your consultant may request additional tests for additional costs, before they are able to provide a personalised treatment plan.
We will provide advice on various treatment options to manage your condition, however there is no guarantee that this will result in a pregnancy following fertility treatment or lead to complete resolution of your symptoms following treatment of gynaecology conditions.
If we believe that if the treatment you request is not in your best interests or is not advisable due to your circumstances, we recommend that you do not proceed with your requested treatment, and we politely decline the treatment through JIVA Fertility.
Please be aware that there may be circumstances when we may withdraw our services and include but shall not be limited to your becoming abusive or threatening towards any of our staff. You may not be eligible for any refund.
Our responsibility
We are responsible to you for foreseeable loss and damage caused by us, but we are not responsible for any loss or damage that is not foreseeable. Loss or damage is foreseeable if it is obvious that it will happen or if, at the time the agreement was made, you and us both knew it might happen.
We do not exclude or limit in any way our liability to you where it would be unlawful to do so. This includes liability for death or personal injury caused by our negligence or the negligence of our employees or subcontractors; for fraud or fraudulent misrepresentation; for breach of your legal rights in relation to the services including the right to receive services which are supplied with reasonable skill and care.
Fertility Assessment Packages
At JIVA Fertility, we have created various, comprehensive Fertility Assessment Packages to help you, as individuals and couples, to understand your reproductive health and to enable you to make informed choices. We have also created packages for assessment of Polycystic ovary syndrome (PCOS) and Recurrent Miscarriage.
Who should consider Fertility Assessment?
- If you are planning to start/extend your family
- If you are experiencing difficulties conceiving
- If you are considering fertility preservation (freezing gametes due to social or medical reasons)
- You merely wish to know about your fertility health.

What is included?
All the Fertility Assessment packages have the descriptions of what is included in them – please read them carefully and ask us any questions you may have before booking any appointment.
Who is eligible?
Any of our patients can make use of these packages at any point during their assessment.
How does these assessment packages work?
Fertility assessment packages are designed to be accessible and affordable. To make it easier for you, we have created most of our packages as ONE-STOP assessment packages, so that you can get answers on the day of your appointment with our Consultant.
You do not need a referral from your GP to book an assessment. Once you book the appointment or register your interest either online through our website or using our WhatsApp number, our dedicated team will contact you to ensure that the assessment package is appropriate for you. We will also help you choose the package that best suits your circumstances, advise you of the costs and arrange an initial visit for the investigations*.
Once all investigations complete (generally two weeks later, but some may take longer), we will offer you an appointment with our consultant Mr Harish Bhandari who will obtain a thorough medical history, undertake appropriate assessment, and, for most of you, he will be able to discuss further management options with you on the day of your appointment.

What is Our Cancellation Policy?
Please note that if you do not attend your appointment for tests OR your clinic appointment after doing the tests, you will not be eligible for any refund.
If you attend the appointment for tests, but do not attend the clinic appointment, you will not be eligible for any refund. Our consultant will send the test results to you, if complete, summarised in a letter.
If you have not had any tests and wish to cancel/reschedule your appointments with less than 48-hour notice, you will be charged 50% of the package fees.
Your tests appointment and Consultation appointment can be rescheduled to a different date/time, which should be within the next 30 days, by giving us 48 hours’ notice.
Once you attend any appointment, you are not eligible for any refund.
These packages are non-transferable and cannot be combined with any other promotions or discounts.
Can I make an appointment with a consultant without opting for any of these assessment packages?
Yes, certainly you can. Before you decide on having any investigations or if you have already had investigations either at your GP surgery or in a different fertility clinic, you can arrange a consultation to discuss these at your appointment - please see the consultation fees section for the costs
Fertility Treatment:
Pre-treatment blood tests for infection screening must be completed within 12 weeks of egg collection – For certain viral infections, we will not be able to offer treatment and the treatment costs minus infection screening costs will be refunded. For some viral infections, we may be able to refer to an appropriate clinic for treatment and the costs of your treatment may vary, accordingly.
Cancellations:
- In the event that a treatment cycle is cancelled before the commencement of a treatment cycle, a Cancellation fee of £500 is payable.
- If the treatment is abandoned prior to egg collection (e.g. poor ovarian response), a Cancellation Fee of £1500 is payable and the remaining amount will be used towards further treatment cycle.
- If the treatment cycle is cancelled after egg collection (e.g. no mature eggs collected, fertilization does not happen, poor embryo development, no suitable embryo for transfer) £500 will be refunded towards further treatment cycle
Any medication used as a result of cancellation will need to be repurchased in order to allow you to commence a new cycle when appropriate.
Any IVF cycle may be converted to ICSI during the treatment cycle as per the sperm quality on the day and will incur an additional charge which will depend on the IVF Clinic where you will be having your procedures.
If embryos are stored for medical reasons as determined by the Consultant or the IVF clinic (e.g. OHSS risk), JIVA Fertility will provide ONE frozen embryo transfer treatment cycle without any fees, however, you will have to pay for the cost of medication.
If mature eggs have to be frozen either due to male partner unable to produce semen sample or if there is no sperm in the semen sample (azoospermia), you are liable to pay for the subsequent ICSI treatment using partner/donor sperm, and frozen embryo transfer treatment cycle. In some circumstances, we will advise you to freeze sperm (at extra costs) before thawing of eggs for treatment.
Eggs/Embryo storage after the initial one year will be invoiced directly by the IVF Clinic where the eggs/embryos are stored.
Concerns and complaints
Our goal is to provide a positive and supportive experience for everyone undergoing fertility treatment, regardless of the treatment outcome. We are committed to providing the best possible care and ensuring that you feel supported throughout your journey with us.
If you wish to share feedback, both positive and negative, we are here to listen.
If you have a concern, we encourage you to speak to us straight away. We will do our best to put things right for you if your experience is less than what we aspire to give.
If your concern is more serious, you can share your complaint directly with us in either by speaking to us or by emailing or writing to us – the contact details are on the website. We encourage you to raise this as soon as possible and include as much detail as possible – your name, date of birth, a detailed account of the events, including the identities of the staff involved, and actions already taken. Please share your thoughts on what actions could be taken to rectify the situation and restore harmony. Kindly also inform us of your preferred method of communication so that we can ensure seamless and effective interaction.
Please note that a complaint raised few months after the event may be difficult for us to investigate fully.
If you wish to raise a complaint on behalf of someone else, for confidentiality reasons, we will need their written consent before we can discuss any of their information with you.
Others
We may transfer this agreement to someone else. We may transfer our rights and obligations under these terms to another organisation. We will contact you to let you know if we plan to do this. If you are unhappy with the transfer, you may contact us to end the agreement within 48 hours of us telling you about it and we will refund you any payments you have made in advance for services not provided.
This agreement is between JIVA Fertility and you. No other person shall have any rights to enforce any of its terms.
If a court finds part of this agreement illegal, the rest will continue in force. Each of the paragraphs of these terms operates separately. If any court or relevant authority decides that any of them are unlawful, the remaining paragraphs will remain in full force and effect.
These terms are governed by English law, and you can bring legal proceedings in respect of the services in the English courts. If you live in Scotland, you can bring legal proceedings in respect of the services in either the Scottish or the English courts. If you live in Northern Ireland, you can bring legal proceedings in respect of the services in either the Northern Irish or the English courts.
